How to Register

A NHS Practice Registration form can be collected from any surgery during opening hours. You will need your NHS number which you can obtain from your previous surgery and two forms of ID (photographic and proof of address – see list below).

If your address does not fall within the Practice area (Looe, Lansallos, Pelynt, Lanreath, Polperro, Widegates, Duloe, St. Keyne or Trewidland – see catchment area map) you will be advised of alternative surgeries covering your address area.

You will be invited for a health check with a health care assistant; this will include a blood pressure test, a urine test and a medical history check. Your registration will be processed and medical records obtained from your previous practice.  You will need to see a doctor for your first prescription.

Approved forms of identification include:

  • Passport
  • Driving licence
  • Utility bill
  • Bank statement
  • Council tax statement
